The following attempts to update the 'Prague pubs' section of Evan Rails 'Good beer guide Prague '.
Of the pubs he listed I have noted any significant changes I am aware of. I have also added a long list of other pubs that you might be interested in. It is not possible to list every pub selling Svijany or Bernard, as there are simply too many to list nowadays.
First of all, a few updates to the venues he listed.
Page 181 'Beograd '.
Now 'Hooters' - avoid!
Page 181 'Ferdinanda'.
Note, now has a sister pub at Karmelitska 18 in Mala Strana. This one sells the 11° and 12° light, the 12° dark and the 13° Sedm Kuli.
Page 184 'Malostranska Pivnice'.
Delete any mention of old fashioned prices, it is now standard tourist area rip-off.
Page 186 'Bastard'.
No longer sells Herold, now a Pilsner pub.
Page 187 'Divci Skok'.
No longer sells Herold, it sells Svijany instead.
Page 187 'Klasterni pivnice'.
Now also sells a rotating beer from another brewery on a third tap.
Page 189 'Plzensky Sklipek'.
Now called U Veverky, one of THE places to drink Pilsner in town.
Page 191 'Poja'.
This place usually sells a strong beer from Jezek as well as Budweiser Burgerbrau. Closed on Sundays.
Page 192 U Radnice'.
Now called 'Svet piva'. As Podkovan no longer exists it now sells a wide variety of Czech and foreign beers on tap. It also has a huge range of bottled beer. Has probably the most 'out there' range of beers in the capital. It also has a small back room bar reached by a separate entrance (to the right of the main bar) which sells around 4 beers, when I was there it included Kacov and Trutnov beers.

I really have no idea why this pub was in the original guide. Anyway it has now changed name to Mlsnej Kocour and only serves Kozel and tank Pilsner.

Considered by many beer connoisseurs to be the best pub in Prague . Usually around fifteen beers from around the country on draught, topped up with a few from Belgium , Germany and the UK . Large selection of bottled beer too. Kacov 10° is the daily drinker beer. Ignore the locals only Staropramen bar at street level, and take the stairs down to the basement where the real quaffing takes place. The bar itself can get quite smoky at busy times, but there is a no smoking area halfway down the stairs where you can enjoy the beer without a topping of smog. They occasionally commision beers for sale at the pub. Take tram 11 or 18 to namesti Bratri Synku, or tram 6, 7, 24 to Otakarova.

Out in 'Panalak City ' on the Southern edge of town (nearest metro Haje), a sister microbrewery & restaurant to Northern Prague 's U Bulovky. Three excellent regular beers plus specials. The food is highly recomended. For the easily intimidated, the area around the pub may be a bit too much at night, so daylight visits may be safer.

Way out in Strasnice, not far from the metro station is this back street boozer. If the smoke doesn't kill you a stray dart from the pub games area will.

And one for the real hardcore desperadoes, I have found an outlet for Lobkowitz although its a long way out. South of Hostivar (by train on JR221 or end of 22/26 tram routes) there is a big reservoir. There is a popular cycle path along the North bank, and at the South East end of the reservoir there is a beer shack 'obcerstveni 'Zartishi' selling Demon (13%) and Gambrinus. You'll need a map to find it.
